Practitioners' Rights

The following is a statement of your rights to know what to expect when working with our staff.

You Have the Right to:

  1. Have information about our programs and services, staff qualifications and any contractual relationships.
  2. Decline to participate in or work with our disease management program and services for your patients, if the client's contract allows.
  3. Be informed of how we coordinate interventions with treatment plans for individual patients.
  4. Know how to contact the person responsible for managing and communicating with your patients.
  5. Be supported by us when interacting with your patients to make decisions about their health care.
  6. Receive courteous and respectful treatment from our staff.
  7. Communicate complaints and provide feedback to us.
